Practice Revenue Leak Scorecard

Sixteen questions, about three minutes. You get a dollar estimate of what your practice is leaving on the table each year — and the arithmetic behind it, so you can check my work.

Every benchmark below is third-party sourced and linked. The calculation runs in your browser; at the end you'll enter an email, and your answers come to us with it so we can talk about your practice specifically rather than averages.

01 Your practice

Four numbers to anchor the math

Estimates are fine. If you leave one blank I'll use the industry average from the Chiropractic Economics annual survey and label it as an assumption in your result.

All providers combined. Industry average: 138/week (2025 survey), 111/week (2024).

What you actually bank per visit, not what you bill. Around $68 is typical — derived from the 2025 survey's average collections and visit volume.

Industry average: 7/week (2025), 10/week (2024).

How much of your revenue runs through insurance or Medicare?

This decides whether the claims section applies to you at all.

Do you bill Medicare?

Medicare covers spinal manipulation only — CPT 98940–98942 — and the documentation bar is higher than most practices realize.

02 Leak one

Patients who stopped coming

The profession benchmarks this as Patient Visit Average — how many visits a patient completes before they disappear. It is the most expensive number in your practice and the one least often tracked.

What's your Patient Visit Average?

Industry average: 34 visits (2025 survey), 37 (2024).

When a patient stops showing up, what happens?
Could you pull a list of every patient who hasn't been seen in 90 days?

03 Leak two

Slots that went empty

A no-show costs the full value of the visit and the room stays warm for nobody. The evidence on what fixes it is unusually strong — eight randomized trials, pooled by Cochrane.

Roughly what share of booked appointments no-show or cancel same-day?

Across 1.6 billion outpatient visits, Epic Research put the rate at 6.2% where patients used a portal and 7.9% where they didn't.

How do patients get reminded?
When someone cancels, does the slot get refilled?

04 Leak three

Money you earned and never collected

This is the section where chiropractic is genuinely an outlier. CMS's own audit puts the chiropractic Medicare improper payment rate at 33.6% — against 10.3% for Part B overall. And 95.5% of it traces to insufficient documentation, not bad coding.

Do you know your claim denial rate?
When a claim is denied, what happens next?

On the commercial side, KFF found fewer than 1% of denied in-network claims are ever appealed.

Does anything check a note for the required documentation before the claim goes out?

Medicare wants at least two of the four PART criteria on every visit, plus objective measures showing the patient is still improving. Once they're stable, it's maintenance care and it isn't covered.

When do your notes actually get finished?

05 Leak four

People who tried to reach you

A new patient who calls, gets voicemail, and doesn't hear back is the most expensive kind of lost revenue, because you paid to make the phone ring.

What happens when a new patient calls and nobody can pick up?
Someone lands on your website at 9pm wanting an appointment. What can they do?
How fast does a new inquiry typically get a human response?

In the largest study of response timing, the odds of qualifying a lead fell 21× between a five-minute and a thirty-minute reply.

06 Leak five

What people find before they call

Somewhere between 79% and 84% of patients read reviews before choosing a provider — three independent surveys landing within five points of each other.

How do you get reviews?
Your current Google rating?

In one 2026 survey, 75% of patients said they wouldn't book below 4.0 stars.

Answer at least the questions in sections 2 through 6 and I can give you a real number.

Where should I send it?

Enter your email and the full breakdown opens below — every category, the dollar figure, and the arithmetic behind each one. You'll also get a copy so you can look at it again later.

One email, no phone number, no sales call unless you book one yourself. Your answers are sent along with it — that's what makes a conversation about your practice possible instead of another pitch about averages. Nothing here identifies a patient, and none of it is sold or shared.

Want the real number instead of the estimate?

The Practice Recovery Analysis replaces every assumption above with your practice's real numbers: a 30-minute kickoff, your EHR's summary reports (totals only, no patient names), and a written recovery plan within 5 business days, followed by a review call. $750, credited toward setup if you move forward within 14 days of receiving the plan.

How this was calculated · every source

This is an estimate built from published benchmarks and your own answers, not an audit of your books. It is deliberately conservative: where a source gave a range, the low end was used.

What we keep: the email, name and practice name you entered; the totals you were shown; and your answers to the sixteen questions above. The math itself runs in your browser — we receive the answers so that if we do speak, we can start from your practice instead of re-asking everything you just filled in. These are operational questions about a business: visit volume, collections, booking, billing. Nothing here identifies a patient and none of it is clinical. We don't sell or share it. To have your record deleted, email als@alsprofessionalservicesga.com and it's gone.
